The dataset details the types of services that could be delivered as part the Transitioning from out of home care: support for aboriginal young peopleof the initiative, which include:

as part of the young person’s care team identify post-care support needs and how they will be met consultation with young person regarding their identified short, medium and long term independent/interdependent living goals facilitate transition and post care brokerage to meet identified transition support needs for those young people on custody or guardianship orders from 16 – 21 years of age assist young people to access specialist support services such as Post Care Support, Information and Referral services, Springboard: Intensive education and employment support for young people leaving care, and Zero Tuition Fees initiative for accredited training.
